S状結腸・直腸進行癌に対する腹腔鏡下手術の手技のポイント

Laparoscopic procedure for advanced sigmoid colon and rectal cancer

◆要旨：S状結腸・直腸進行癌に対する腹腔鏡下手術に必要な解剖学的要点と手術手技を紹介する．筆者らは下腸間膜根リンパ節（253）の郭清を左結腸動脈（LCA）を温存する方法で行っている．本法は口側腸管の血流維持などの面で優れており，良好な成績を示している．ただ，S状結腸・直腸進行癌に対する腹腔鏡下手術は本邦においては現段階では開腹手術との比較試験が進行中であることを認識する必要があり，その結果が待たれる．

　In this report, the importance of anatomy and surgical procedures for advanced sigmoid colon and rectal cancer is introduced. Lymph node resection of No. 253, preserving left colic artery from inferior mesenteric artery, is performed in our procedure to maintain the blood flow to the oral side of the bowel. However, randomized control study comparing the clinical outcomes between open and laparoscopic surgery for advanced colorectal cancer is still in progress in Japan and its result should be considered as well.
