体内でのループ作成を不要とした新しい内視鏡下体内結紮法

New intracorporeal knot using pre-looping and slip knot tying technique for endoscopic surgery

内視鏡下手術の普及に伴い，体内結紮が行われることも増えてきた．内視鏡下手術ではワーキングスペースが十分に得られない場合，体内での鉗子操作によるループ作成に手間取ることも多く，結紮糸の把持の際，余分な力が加わりやすいため結紮が緩みやすい傾向がある．今回，あらかじめ体外で二重のループを作成しておきslip knotテクニックを用いて結紮する，新しい内視鏡下体内結紮法の一方法を考案したので報告する．本法は，体内でのループ作成が不要なため，鉗子操作は直線的な動きのみでよく，閉め具合の調節が可能であること，通常使用する内視鏡下手術用器具で可能な手技であることなどの利点がある．

　Recently the number of endoscopic surgery has increased, and intracorporeal knot-tying technique is considered a basic skill for surgeons. However, performing this task thorachoscopically or laparoscopically can be a tedious, time-consuming endeavor associated with much frustration.

　We describe herein a new intracorporeal knot-tying technique for endoscopic surgery, using pre-looping and slip knot tying technique, in an effort to make the procedure simpler. In this procedure, hand/finger movements like rotating or twisting for making loops are not necessary, sliding knots for good sutures are easy, and we do not need extra components.
