胸腔鏡下後縦隔腫瘍切除術

Video-assisted thoracoscopic excision for masses of the posterior mediastinum.

　一般に後縦隔腫瘍は開胸術により切除されてきた．後縦隔腫瘍の大部分は，良性で無症状であり，偶然発見されることが多く，切除は比較的容易である．このような点から，より侵襲の少ない術式が望まれていた．最近のビデオ胸腔鏡の導入および関連機器の発達・普及により，多くの胸腔内疾患に対する胸腔鏡下の検査・治療が急速に普及したが，胸腔鏡下後縦隔腫瘍切除術もその1つである．本稿では、後縦隔腫瘍に対する胸腔鏡下切除術の適応と成績について解説する．

Posterior mediastinal masses are generally excised through wide thoracotomies. These lesions are ofen benign, usually asymptomatic, discovered incidentally, and relatively easy to resect. For these reasons, a minimally invasive approach is appropriate. Recent advances in endoscopic equipment, refinement of thor-acoscopic techniques, and increased experience with thoracoscopy have expanded the application of this procedure for masses of the posterior mediastinum. This report describes present role of video-assisted thoracoscopic surgery for masses of the posterior mediastinum.
