重症虫垂炎に対する腹腔鏡下手術—医療経済の面から

Laparoscopic Surgery for Appendicitis with Severe Inflammation: A Clinical Study with Cost Analysis

　開腹による虫垂炎手術症例156例の炎症所見，入院期間，合併症について検討したところ，穿孔性虫垂炎では有意に入院期間が長く，創感染，腹腔内膿瘍の頻度が高かった．この予備研究の結論から，腹腔鏡下手術の小さい創，広い視野と良好な洗浄性が有効と考え，対象を重症虫垂炎に限定し，腹腔鏡下手術5例と開腹手術5例の臨床経過と医療費を比較した．腹腔鏡下手術では，ドレーン抜去期間，術後入院期間が有意に短く，創感染などの合併症もなく，quality of lifeに優れていた．医療費については，腹腔鏡下手術では入院料は有意に低額であったが，手術料は高額であり，総医療費としては差はなかった．重症虫垂炎に対しては，腹腔鏡下手術が臨床的に有用と考えられた．

From clinical outcomes of our 156 cases with open surgery for appendicitis, it was clearly found that a mean hospital stay was significantly long and postoperative wound infection and intra-abdominal abscess were more frequently occurred in cases of perforative appendicitis. Considering the advantages of laparo-scopic procedure such as, wide view through a small wound and easy management for lavation of abdominal cavity, this procedure seemed to be effective for appendicitis with severe inflammation.
