Video-assisted Mitral Valve Surgery

　われわれが積極的に行っている低侵襲小切開心臓手術（minimally invasive cardiac sur—gery：MICS）のなかでも特に内視鏡支援下で行う僧帽弁手術（video-assisted mitral valve sur—gery）のMICSについて記した．慶應義塾大学医学部外科で，1998年5月までにMIDCABを除く50例にMICSを施行した．そのうち内視鏡支援下で行った僧帽弁の手術は22例である．手術成績は，手術死亡，遠隔死亡ともになく，内視鏡支援下でのMICSが有用であり，安全に施行できることが立証できた．これらのアプローチにより，患者のQOLの向上と迅速な社会復帰を促進し，入院期間の短縮，医療費の削減をはかることができる．これらの手術は，今後，工学的支援の進化に伴いより標準化されていくことが期待される．その1つにHeartport社のportaccess systemによるvideo-assistedの僧帽弁手術があり，今後の展開が期待される．このような低侵襲化による治療法は，医学的必然であると同時に社会的要請でもある．

We report video-assisted mitral valve surgery using minimally invasive cardiac surgery “MICS” which is positively adopted in our Surgery Department of Keio University. We have performed MICS on 50 cases excluding MIDCAB till May 1998. Twenty-two case out of 50 were video-assisted mitral valve surgery. The results proved that the video-assisted MICS is effective and can be performed safely with no operative or remote death. This approach improves the QOL of patient and facilitates earlier rehabilitation, reducing hospitalization period and medical cost.
