腹部外傷に対する腹腔鏡検査・手術—腸管損傷に対して

Laparoscopic Diagnosis and Treatment for Bowel Injuries

　腹部外傷，とりわけ腸管損傷に対する腹腔鏡診断および補助下手術の有用性と安全性について検討した．過去4年間に，腹部所見から腸管損傷が疑われた25例（全身状態の安定した症例）に対して，全身麻酔下に腹腔鏡を施行した．1例は，気腹直後に血圧低下がみられたので開腹に移行した．24例中11例に小腸損傷を確認し，6例は開腹で，5例は小開腹で損傷部を修復した．13例は腹腔鏡で腸管損傷がみられなかったので，保存的に治療した．術後，消化管損傷の見落としによる合併症は認められなかった．腹腔鏡（補助下手術）の導入で腸管損傷の診断能は向上し，不要な開腹は回避でき，腸管損傷に対する小切開手術も可能となった．

To determine the usefulness and safety of laparoscopic diagnosis for bowel injuries, 25 hemodynamically stable patients with suspected bowel injury were evaluated laparoscopically during the past four years. One case with penetrating trauma was converted to open surgery because of hypotension just after pneumoper-itoneum. Of the 24 patients, 11 had small bowel injuries. Repair of the bowel injuries were performed through minilaparotomy in five patients. The remaining thirteen patients had no bowel injuries, and did not undergo laparotomy.
