直腸病変に対するEMRおよびTEMの現状と展望

The Present Situation and a Prospect of EMR and TEM for the Rectal Tumor

　直腸の腺腫，m癌やsm₁癌の治療はEMRが第一選択であり，腫瘍径が20mm程度までは一括切除とする．それ以上でも分割的にEMRは可能だが，30mmを越えるものは困難である．一方，TEMはEMRより侵襲が大きいが，腫瘍径にかかわらず一括切除が可能である．しかし，リンパ節郭清ができないので壁深達度における適応はEMRと同様である．すなわち，10％前後のリンパ節転移を認めるsm²〜sm₃は，EMRならびにTEMの適応とはならない．現状では分割切除と一括切除の間に明らかな優劣はないので，EMRができない集簇様病変などがTEMの適応と考えられる．将来，sm癌やmp癌のリンパ節転移の有無が高率に予測できるようになれば，全層切除ができるTEMの適応は拡大されるであろう．

EMR is the treatment of first choice for polyps, as well as for m and sm₁ cancer of the rectum. EMR is indicated for removing tumors up to about 20 mm in size by en bloc resection and even for removing larger tumors by piecemeal resection. However, it is hard to use for lesions exceeding 30 mm in size. TEM is more invasive than EMR, but it allows en bloc resection of tumors regardless of size. However, lymph node dissection is not feasible, so TEM is only indicated for lesions with a depth of invasion similar to those treated by EMR.
