EMR is the treatment of first choice for polyps, as well as for m and sm1 cancer of the rectum. EMR is indicated for removing tumors up to about 20 mm in size by en bloc resection and even for removing larger tumors by piecemeal resection. However, it is hard to use for lesions exceeding 30 mm in size. TEM is more invasive than EMR, but it allows en bloc resection of tumors regardless of size. However, lymph node dissection is not feasible, so TEM is only indicated for lesions with a depth of invasion similar to those treated by EMR.
