14．免疫異常に対する治療戦略—免疫抑制治療の有害事象と使用時の注意点

新しい機序の免疫抑制剤や癌治療が実用化され，多くの疾患の生命予後や生活の質（QOL）が改善した。その一方で，医原性に免疫抑制状態を呈する患者の数は増え続けている。免疫抑制治療中の患者が集中治療管理を要することは少なくないので，集中治療医も治療薬の作用機序や有害事象を把握していることが望ましい。本稿では，まず膠原病における免疫抑制剤の使い方の大枠もまとめる。次いでステロイドの使い方を詳述し，他の免疫抑制剤や生物学的製剤，JAK阻害薬の代表的な有害事象を概説する。免疫異常につながる悪性腫瘍の治療として，mTOR阻害薬，プロテアソーム阻害薬，免疫チェックポイント阻害薬，CAR-T療法にも言及する。

The development of immunosuppressants and cancer treatment with new mechanism of action has improved both the prognosis and quality of life of patients with various diseases；however, it has also led to an increase in the number of patients under iatrogenic suppression of the immune system. Because patients taking immunosuppressive drugs often require intensive care for severe infections and other associated issues, intensivists should understand both the mechanism and side effects of immunosuppressive treatment. In this review, synthetic glucocorticoids, immunosuppressants, biologics, JAK inhibitors and cancer treatments that cause iatrogenic immunodeficiency are reviewed. The principal strategy for treating autoimmune diseases is also discussed.
