抗HIV療法（ART）を開始したあとに，日和見疾患の症状が悪化することがあり，免疫再構築症候群（IRIS）と呼ばれている。IRIS発症時には，ARTと原疾患に対する治療を継続しつつ，必要に応じて炎症に関連する症状に対する治療を行う。IRISは，通常は一過性であり予後は良好であるが，生命予後の悪化や重篤な機能障害につながる場合がある。ART開始前の日和見疾患スクリーニングが重要である。

Paradoxical worsening of pre-existing opportunistic disease after initiation of antiretroviral therapy （ART） is known as immune reconstitution inflammatory syndrome （IRIS）. Although most cases are mild and self-limiting, severe IRIS can be life-threatening. Screening for opportunistic diseases prior to initiation of ART is thus important.
