12．免疫再構築症候群（IRIS）の免疫学—non-HIV患者でみられるIRIS：その多様な病態の臨床的特徴

免疫再構築症候群（IRIS）は，これまでHIV（ヒト免疫不全ウイルス）感染症患者にしかみられないと考えられてきたが，近年non-HIV患者でも似た病態がみられることがわかってきた。non-HIV患者では，免疫抑制剤の中止や減量，免疫チェックポイント阻害薬の導入，好中球減少からの回復，出産などを契機に宿主の免疫が急速に賦活してIRISをきたす。HIV患者のIRISと異なり，背景や発症機序が多様であることから，概念そのものや診断基準，治療法がいまだ確立していないことが問題である。今後，non-HIV患者におけるIRISの症例を集積し，疾患概念とともに診療指針が確立することが望まれる。

Immune reconstitution inflammatory syndrome （IRIS） was previously thought to occur only in HIV patients. However, it is now known that a similar condition occurs in non-HIV patients. Unlike IRIS in HIV patients, the concept itself, diagnostic criteria, and treatment have not yet been established for IRIS in non-HIV patients because the backgrounds and pathogenesis of IRIS in this particular population are diverse. The accumulation of more IRIS cases in non-HIV patients in the future will help establish the disease concept and guidelines for its treatment.
