Time-limited trial and shared decision making for continuous renal replacement therapy in patients with acute kidney injury

AKIという診断基準が誕生して以来，基礎研究，臨床研究や医療技術の開発など，critical care nephrologyの領域が目覚ましい発展を遂げている。しかしながら，医療が発展したからといって，すべての重症患者を救命できるわけではない。そのため，時として持続的腎代替療法（CRRT）が患者のQOL（生活の質）を下げるだけの治療になることがある。そのような場合には，共同意思決定shared decision makingを通じて，患者・家族と医療者間で密にコミュニケーションをとり，意思決定を行うことが望ましい。また状況に応じて，time-limited trialで，一定の期間ごとに治療の目標を設定し検討することも有用である。

Since the birth of the diagnostic criteria for AKI, critical care nephrology has made remarkable progress in both basic and clinical research and also in the development of medical technology. However, despite the development of medical care, we still cannot save all critically ill patients. Therefore, CRRT could unexpectedly impair the patient's quality of life. In such cases, it would be desirable to communicate with the patient, their family, and medical staff through a shared decision-making process. Time-limited trials are also helpful for considering and setting treatment goals for a certain period of time.
