time-limited trial（TLT）とは，患者・家族と医療チームがお互いに話し合ったうえで，予後や治療反応性が不確実である間は特定の治療や介入を一定期間行い，あらかじめ設定したゴールが達成できそうであれば治療を継続するが，ゴールが達成できないことがある程度明確になった場合はゴールを完全緩和に変更して延命治療を中止する，一連の意思決定プロセスである。TLTによって急性期診療におけるさまざまなジレンマを解決できる可能性がある一方，患者・家族に対して望ましくない効果がもたらされる可能性もある。本稿は，TLTを深く理解し，適切に用いることができることを目的としている。

Time-limited trial （TLT） is a series of decision-making processes in which the use of specific treatments or interventions for a certain period of time are proposed to a patient or his / her family after mutual discussions while the prognosis and response to treatment are uncertain. Treatment continues if the predetermined goal is likely to be achieved, or the goal is changed to complete palliation if it becomes more clear that the goal cannot be achieved. TLT has the potential to solve various dilemmas in acute care, but it may also have undesirable effects on patients and families. The purpose of this article is to provide an in-depth understanding of TLT for its appropriate use.
