critical care nephrologyは集中治療を行ううえで重要な概念の1つである。集中治療のチーム医療のなかで，この領域について薬剤師に求められることは，薬物投与量の適正化である。そのためには，腎機能低下時に投与量の調整が必要な薬物と薬物投与設計の実際，腎機能評価の基本と集中治療における応用，持続的腎代替療法（CRRT）におけるクリアランスの考え方を理解する必要がある。臨床薬理学や薬物動態学の知識が求められる大変重要な役割である。本稿では，薬剤師のcritical care nephrologyへのかかわりについて，薬物投与量の適正化を中心に概説する。エビデンスが不足する本領域での，薬剤師のさらなる活躍に期待したい。

Critical care nephrology is an important concept in intensive-care medicine. In the intensive-care unit, pharmacists are required to optimize drug dosage and form. To achieve this goal, they need to understand the characteristics and appropriate dosage adjustment for each drug according to the renal function. In addition, pharmacists need to have a basic knowledge of the renal function assessment and its application in intensive care, along with an understanding of the concept of clearance in contentious renal replacement therapy. This is a very important role that requires knowledge of clinical pharmacology and pharmacokinetics. This article outlines the pharmacist's involvement in critical care nephrology with a focus on drug dosage optimization. We hope that pharmacists will play a more active role in this field in the future.
