災害は，通常の機能を破壊し，地域の対処能力を超えた苦痛と喪失を引き起こす。災害時には，災害現場でのトリアージと救急部門でのトリアージのあとに，重傷者の行き先がICUとなり，災害時のICU看護師の役割が重要になる。災害時のICUでは，重篤な患者が災害現場から直接，またはICUのない施設からの広域搬送によって移送されることに加え，手術患者の管理も従来どおり行われるため，さらに重要になる。そのため，災害時のICU看護師は状況に応じて，災害時のICU入退室基準を変更し，日常の看護とは異なる対応をする必要がある。多数傷病者に対応するICUの機能には，surge capacityとsurge capabilityがある。これらの能力を拡大するためには，日々の準備や基準を整備する必要がある。
