⑥顔面外傷—換気不能，酸素化不能の危険性を念頭に，外科的気道確保をためらわない

顔面外傷の症例の気道確保では，まず，口腔内吸引による気道のdecontaminationをはかる。気道が開通され換気可能となれば時間的余裕が生まれるが，持続する口腔内出血と下顎骨骨折により換気困難に直面するであろう。換気困難となれば，声門上器具を試行することになるが，下顎骨骨折があれば固定も不十分となり，また口腔内出血により気道のdecontaminationがやはり困難であるため，十分な気道確保になり得ない。最終的に外科的気道確保をためらわず，迅速に行える用意と心構えが必要である。

First, suctioning of massive oral bleeding should be attempted. If ventilation is possible after suctioning, there is enough time to continue ventilating. However, persistent bleeding and anatomical instability may make mask-ventilation impossible. If the “cannot ventilate” situation develops, a supraglottic airway should be placed, but that can't establish an adequate airway and oxygenation due to mandibular instability and a contaminated airway. Accordingly, this situation demands preparations for rapid cricothyroidotomy.
