Massive hemoptysis is a potentially life-threatening condition, which requires timely action. The critical initial step in the management of patients with massive hemoptysis is establishing an airway followed by prompt diagnosis and a definitive approach to hemostasis. When considering the use of bronchoscopy with a bore aspiration lumen, we recommend the largest single lumen endotracheal tube for airway control. Although a double lumen tube is commonly used for independent lung ventilation, it is not recommended for initial airway control. The endotracheal tube should be selected for operability of a large-sized bronchoscope and secure ventilation. We do not recommend rapid sequence intubation for patients with massive hemoptysis. In this article, we review the diagnostic evaluation, tube selection, and bronchial blocking technique for patients with massive hemoptysis.
