Aiming for full feedings in patients with ARDS

急性呼吸窮迫症候群（ARDS）における栄養療法の重要性は，他の重症疾患と同様に理解されている。一方で，適切な投与エネルギーや投与時期に関しては，解明できていない部分が多い。ここでは，過去に行われた無作為化比較試験を中心に検証した。急性期における栄養療法では，full feedingを目指すことの有用性は示されていない。加えて，trophic feedingによる飢餓も考慮し，現状患者がどれほどの重症度にあるのかを考え，栄養設計しなければならない。

In treating patients with acute respiratory distress syndrome （ARDS）, the importance of nutritional therapy is understood as well as in other critical illnesses. The optimal caloric needs and timing of feedings are not clear. We assess these factors based on past randomized controlled trials. Nutritional therapy to aim for full feedings in the acute phase has not been shown to be effective. The risk of starvation by trophic feedings should be considered. We should consider nutrition design based on the patient's current status.
