Postintensive care syndrome (PICS) may develop or be exacerbated in patients with longer periods of delirium, immobilization, mechanical ventilation, and intensive care unit (ICU) stay. These aggravating factors can be modified with the ABCDEFGH bundle, which may reduce the burden of PICS on critically ill patients and their families after discharge. The ABCDE bundle can reduce delirium, ICU-acquired weakness and PICS with no additional adverse events including self-extubation. Recently, FGH elements, including family involvement or follow-up referrals or functional reconciliation, good handoff communication, and handout materials on PICS and PICS-Family, have been added to ABCDE elements. The FGH bundle can be used after discharge from an ICU to general wards or other facilities to further improve the patient's and family's condition. FGH elements may facilitate shared decision-making with patient and family, continuity of care by adequate handoffs and referrals, and memory reconstruction with support documents such as an ICU diary. A short-term goal for patient discharge from the ICU is important, but improvement in the long-term prognosis and quality of life in patients after discharge from the hospital with the ABCDEFGH bundle is more essential.
