細胞外イオン化Ca(Ca2+o)濃度は厳密に制御されており,その要となる分子がCa感知受容体(CaSR)である.CaSRは副甲状腺,腎尿細管をはじめ種々の臓器で発現しており,Ca2+oに応じて臓器特異的生理作用を発揮し,Ca恒常性を維持する.CaSRの機能亢進,機能低下をきたす病態の発見によりCaSRの生理作用に関する理解は深まったが,シグナル伝達機構の詳細など未解明の領域もあり,CaSR作動薬の開発・応用とともに今後の研究が期待される.
The calcium-sensing receptor (CaSR) plays a major role in maintaining a constant level of extracellular ionized calcium (Ca2+o) concentration. Inherited and acquired disorders of Ca2+o sensing have greatly contributed to our understanding of the physiology and pathophysiology of the CaSR. However, areas such as the critical pathway through which the CaSR mediates its biological effects are still unknown. Filling in these gaps should enable us to modulate CaSR activity in tissue specific and therapeutically desirable ways.
