The calcium-sensing receptor (CaSR) plays a major role in maintaining a constant level of extracellular ionized calcium (Ca2+o) concentration. Inherited and acquired disorders of Ca2+o sensing have greatly contributed to our understanding of the physiology and pathophysiology of the CaSR. However, areas such as the critical pathway through which the CaSR mediates its biological effects are still unknown. Filling in these gaps should enable us to modulate CaSR activity in tissue specific and therapeutically desirable ways.
