口腔・咽頭異物（ヤリイカ精子嚢）の1症例

Foreign Body of the Oral Cavity and the Oropharynx (Sperm-bags of Squid)

I．はじめに

　口腔は咽頭・食道などと比べてその内腔は解剖学的に広く，食物を一時貯留し咀嚼・嚥下するという機能の点から異物が介在しにくい。また直視可能な部位であることから医師の手を煩わさず異物を摘除できるため，この部の異物症例は少ない。日本医科大学耳鼻咽喉科救急外来でも，全異物症例のうち扁桃を含む咽頭の異物症例は比較的多く約40％に達するが，口腔異物は約1％でかなり稀である。口腔異物として食物性異物はあまりなく，数少ないわれわれの施設の過去の例でも非食物性異物がほとんどである。また成人では異物の由来が既知であり，同定が難しい症例はほとんどない。今回われわれはヤリイカの内臓を生食してその精子嚢が口腔・咽頭異物となり，除去は比較的容易であったが外来での即座の同定が困難であり，走査電子顕微鏡などを使い異物を同定した症例を経験したので報告する。

　A 47-year-old male was complaining of pain and foreign bodies sensation in the oral cavity after eating uncooked squid. Spindle shaped foreign bodies were observed in the oral cavity and the oropharynx, and parts of them were struck into mucous membrane. They were at first thought to be parasites because of their similarity in shape and color. Light- and electron-microscopic observations revealed that they were sperm-bags contained sperms of squid.
