鼻・副鼻腔のinverted papilloma—とくに癌合併例，癌化例について

Inverted Papilloma of the Nasal Cavity and Para-nasal Sinuses: Association with Carcinoma

I．はじめに

　鼻・副鼻腔にみられるinverted papillomaは比較的稀な良性腫瘍であるが，臨床上高頻度の再発と癌との密接な関係が指摘されている点で注意を要する。

　当科における過去10年間の集計によれば鼻・副鼻腔粘膜に発生したinverted papillomaは18例あり，そのうち5例（28％）に癌となんらかの関連がみられた。すなわちこれらは1）初回切除時にinverted papillomaと癌が共存していた"癌合併例"の3例（17％）と，2）初回invcrted papillomaとして治療され再発腫瘍が癌腫と診断された"癌化例"の2例（11％）であった。今同はとくにこれら癌との関連がみられた症例について臨床経過と病理組織を検討し報告する。

　Inverted papilloma is an uncommon benign tumor involving mucous membrane of the nasal cavity and paranasal sinuses. Clinically, high frequency of postoperative recurrence and occasional association with malignancy have been documented.

　During the past decade, there were 18 cases of inverted papillomas in our clinic. Of these, 5 cases (28%) were associated with squamous cell carcinoma ; in 3 cases, squamous cell carcinoma coexisted with inverted papillomas, and in 2 cases, inverted papilloma recurred as squamous cell carcinoma 8 years and 5 months after initial treatment in respective cases.

　Importance of radical excision and long term follow-up are stressed.
