I.はじめに
欧米でのCawthorne1),Derlacki2,3)らの報告に続き,近年わが国においても中耳先天性真珠腫の報告は比較的多くみられるようになり,その認識は高くなってきているが,成因などについてはいまだ不明の点も多い。
今回われわれは鼓膜所見正常で耳疾患の既往もなく,手術所見も合わせて先天性真珠腫と考えられる症例を経験したので,若干の文献的考察を加えて報告する。
A 9-year-old boy had complained of hearing impairment for three months with no history of otitis media. On the otoscopic examination, both tympanic membranes were normal. The examination showed a left-sided conductive hearing loss, and x-ray examinations showed no abnormality.
At the operation, we found that the upper part of the posterior compartment and the attic were occupied by a cholesteatoma mass which had no relation with the tympanic membrane. This case was diagnosed as a congenital cholesteatoma.
The study of 15 cases of congenital cholesteatomas (tympanic and/or mastoid type) including the author's case revealed that the majority of congenital cholesteatoma occurred near the tympanic isthmus.
