小児甲状腺癌の1症例

Thyroid Carcinoma in Children

I．はじめに

　小児甲状腺癌はまれな疾患で，本邦では小児科・外科領域からの報告が散見される程度である。小児甲状腺癌は成人例に比して早期に頸部リンパ節転移や肺への遠隔転移を起こし進行した症例が多い。そのため手術療法は拡大傾向にあるが，身体の発育途上にあるため術後後遺症についても十分考慮される必要がある。

　今回われわれは左側頸部腫瘤を主訴として来院した14歳男児の甲状腺癌症例を経験したので文献的考察を加えて報告する。

　A 14-year-old boy with thyroid carcinoma with metastasis to the left cervical lymph nodes was reported. Only 67 children with thyroid carcinoma have been reported in the literature up to 1978 in Japan.

　The clinical pictures of thyroid carcinoma in children were charaterized by early metastasis to the lungs and the cervical lymph nodes. In Europe and U.S.A., there have been many reports of thyroid carcinoma after radiation, however, this patient received no radiation.

　Surgery consisted of left hemithyroidectomy and radical neck dissection. Pathologically the tumor was a papillary carcinoma. Eighteen months after the surgery, no signs of recurrence and metastasis have been found
