 C3a and C5a, fragments of complements most closely related to inflammatory reaction in middle ear fluids, were measured.

 C3, C4, CH50 in serum and C3a, C5a in plasma were normal.

 Extremely high values of C3a and C5a in middle ear fluids was observed.

 The values of C3a were 18,000ng/ml at maximum, 5,80Ong/ml at minimum, and 9,293ng/ml on average. The values of C5a were 86ng/ml, 20 ng/ml, and 55ng/ml, respectively.

 The above results suggested that an extremely intensive inflammatory reaction might take place in the middle ear, and anaphylatoxin might play an important role as a delayed factor in otitis media with effusion.
