鼻閉を主訴とした巨大下垂体腺腫の1例

A Case of Large Pituitary Adenoma with Nasopharyngeal Extension

I．はじめに

　下垂体腺腫のextrasellar extensiollはJefferson^1）によれば「鞍外部が鞍内部に対して少なくとも同じぐらいの大きさか，あるいはより大きい腺腫」と定義されている。その進展方向から1）frontal extension，2）hypothalamie extension，3）parasellar extension，4）temporal extension，5）posterior extension，6）nasopharyngealextensionの6型に分類される。われわれは鼻閉のみを主訴としnasopharyngeal ＆ temporalextensionを示した巨大な下唾体腺腫の1例を経験したので報告する。

　A 15-year-old female patient with a pituitary adenoma complained of nasal obstruction due tonasopharyngeal extension of tumor. CT revealed a large mass extending to the temporal fossa, sphenoid sinus and epipharynx. Pathological examination showed the tumor mostly composed of chromophobe adenoma and partly of eosinophilic cells. ⁶⁰Co irradiation with total dose of 59.4 Gy markedly decreased the size of tumor. Although she presented hypopituitarism after irradiation, she has been well controlled by hormonal therapy.
