I．はじめに

　内耳窓破裂は1968年にFee^1）が外傷症例での卵円窓よりの外リンパ瘻を手術的に確認して以来多数の報告があるが，外傷や耳手術の既往のない特発性内耳窓破裂は上気道感染時の咳，くしゃみや身体労作などの日常動作中に起きることが多く，その成因や病態は興味あるものである。われわれは水頭症をはじめ多数の奇形をもつ成人で，咳嗽発作を誘因とし発症した内耳窓破裂が疑われる症例を経験した。本症は伝音難聴が主な混合難聴を呈し脳脊髄圧の減圧で治癒したので，先天性耳性髄液漏との鑑別を含め若干の考察を加えてここに報告する。

　A 28-year-o1d female with hydrocephalus and multiple anomalies developed a spontaneous perilymphatic fistula after coughing attacks.

　Audiometric examination revealed the patient having pure conductive deafness which returned to normal following treatment of hydrocephalus. A decreased compliance of the brain and the rapid CSF pressure change during coughing attacks were supposed to be responsible for the cause of perlymphatic fistula of this particular case.
