メニエール病症状を呈した外リンパ瘻

Clinical Endolymphatic Hydrops in Case with Perilymph Fistula

　I．はじめに

　手術既往のない外リンパ瘻の存在は1968年にFee^1），が初めて頭部外傷例での卵円窓破裂を報告している。以来1981年までに文献的には134例の外リンパ瘻が報告されている^2）。この外リンパ瘻の臨床像としてはGoodhill^3），の報告した突発難聴が有名であるが，進行性や変動する感音難聴の型をとるものもある。また稀には聴覚・前庭所見が典型的な内リンパ水腫を示すものがあり，メニエール病として治療されていると推定される。

　我々は典型的内リンパ水腫所見を呈した蝸牛窓膜破裂による外リンパ瘻を経験したのでこれを報告し，併せてメニエール病と外リンパ瘻による内リンパ水腫の鑑別診断と蝸牛窓膜に対する外科的到達法と膜の確認法について報告する。

　An unusual case of typical Meniere's disease is presented, and the patient was found at exploratory tympanotomy to have a round window rupture. We feel that important diagnostic acumen to differentiate between Meniere's disease and endolymphatic hydrops with perilymph fistula are history, popping sound, peculiar tinnitus, positive fistula sign and positional nystagmus.
