迷路気腫を伴った外傷性外リンパ瘻の1例

Perilymphatic fistula with pneumolabyrinth by a traumatic injury

Ⅰ．はじめに

　外リンパ瘻の確定診断を得るには，鼓室開放術もしくは内視鏡を用いて外リンパ液の漏出を確認する必要があり，術者の主観に委ねる側面もあって，診断率は報告するものによって大きく異なっている。今回，われわれは画像所見から外リンパ瘻を術前に確定診断できた迷路気腫を経験したので，若干の文献的考察を加えて報告する。

　Pneumolabyrinth of the inner ear is extremely rare. We reported the case of a 37-year-old man with pneumolabyrinth caused by a traumatic injury. He was diagnosed of traumatic perilymphatic fistula by computed tomography scans,which demonstrated air in the vestibule. In exploratory tympanotomy,the stapes was not found and the oval window was closed by temporal fascia after replacing the air with saline. The patient has been free from vertigo,but the hearing loss was not improved. We discussed the mechanism of vertigo and hearing loss in the case of perilymphatic fistula.
