迷路気腫を伴った術後性外リンパ瘻の1例

Pneumolabyrinth：A case of postoperative perilymphatic fistula

I．はじめに

　迷路気腫（pneumolabyrinth）とは，1984年にMafeeら^1）が初めて使用した用語で，外リンパ瘻に伴い内耳に気泡が存在する状態である。本症の原因として，先天奇形^2），外傷^3～7），中耳手術後^1,8～11）の報告があるが，国内では外傷による2症例^12,13）の報告のみである。本稿では，アブミ骨手術後に発症した迷路気腫を伴う外リンパ瘻に対して手術を施行し，著明な聴力改善が得られた症例を経験したので，迷路気腫の特徴について考察しながら報告する。

　Pneumolabyrinth of the inner ear is a rare entity. In this report, we described a case of pneumolabyrinth resulting from stapes surgery, and cured by surgical treatment. A 26-year-old man who experienced left-sided severe hearing loss and vertigo one week post stapes surgery. The diagnosis of pneumolabyrinth was established by the detection of air bubbles in the vestibule using high-resolution computed tomography. In exploratory tympanotomy, we replaced the air bubble with saline. After the operation, the hearing loss was recovered to the normal level. We discussed the mechanism how the air bubbles entered into the labyrinth, the cause for the hearing loss and the effect of surgical treatment.
