 A 51-year-old female was operated on for meningioma in the left middle fossa. At that time, the pneumatic cells of the temporal bone were opened. They were closed completely during the procedure, and there was no liquorrhea clinically. Pneumocephalus was caused by Eustachian tube catheterization for secretory otitis media in the left ear. Massive air in the subcutaneous, subdural and dead space were observed on CT scan 19 days after craniotomy.
