耳下腺の二重撮影法

Sialography Using Double Radiographic Technique

I．緒言

　耳下腺疾患の診断法には視診と触診など一般検査のほか機能検査法と画像診断法とがある。このうち後者は器質的変化の検索が主目的で，唾液腺造影法をはじめ断層撮影法とCTとがある。これらの適用は症例に応じて決定されるが，そのなかでは唾液腺造影法の利用が多い。その理由として本法は実施が簡便であるほかかなりな診断情報が得られることが挙げられ，耳下腺の検査法としては第一選択である。

　この場合耳下腺では浅葉と深葉間を顔面神経が走行する関係で，手術に伴う本神経の副損傷防止のためにもとくに病変部位の特定が重要である。したがって唾影像による診断には側面像とともに前後像が欠かせない。これによって主排泄管の内方あるいは外方への偏位の有無を確かめ，主病変が顔面神経走行面の外方，あるいは内方にあるかを間接的に把握できて有利である。しかしここで問題になるのは前後像では唾影像の一部に下顎と側頭骨の重複が避けられないことである。通常この骨との重複部とそれ以外の軟組織部を同一濃度で描出するのは困難である。この解決のために撮影条件を変えた二回撮影法のほか種々な方法が行われている。そこで濃度の異なる両部位の同時描出を目的に，ここではおのおのの部位に適した増感紙とフィルムの組合わせを選択し，この2系列を直ね合わせ1回曝射にて2枚の画像を作成した。この二重撮影法は耳下腺前後像の描出に優れていたのでその概略を報告する。

　In the anteroposterior view of the parotid sialography, there are marked differences in density between the superficial and deep portion of the gland because the bony structure was superimposed in the deep portion. For this reason, a single routine exposure may result in obscuring of depiction of the more dense area. In an attempt to eliminate this, double radiographic technique utilizing two kinds of intensifying screens and x-ray films in the same casette and the routine exposure were used, and the better depiction of the parotid ductal system was obtained. This technique is valuable to evaluate a wide view of pathlogic process of the parotid gland.
