長期鼓室内留置用チューブの使用経験

The Long-term Result of Middle Far Ventilation Tube

I．はじめに

　滲出性中耳炎に対する鼓室内チューブ留置術（以後tubingとする）は1954年Arrnstrong^1）によって発表され，以来その優れた治療効果から現在のところ最も有効な治療法の一つに挙げられている。しかし従来より使用されてきたチューブは自然脱落が多く，治療に必要な充分な期間留置することが難しいという欠点があった。

　われわれは昭和56年4月から長期鼓室内留置を目的として考案されたGoode氏T-tube（Xomed社製）を141耳に使用した。今回これらの症例を検討しT-tubeによるtubingの効果，合併症，管理上の問題点，治療に必要なtubingの期間について考察する。

　The results of three year follow up studies on 141 ears with otitis media with effusion treated by the long-term ventilation T-tube were reported. Complications occurred in 39% of the ears. The most common complication was infection which was found in 38%. In 46% of the infected ears, the tube was inevitably removed, but spontaneous extrusion was very rare.

　Hearing was improved in 71%. In the ears with retracted and atelectatic ear drum hearing restoration was excellent. Hearing improvement in the ear with adhesive drum or cholesterin granuloma was unsatisfactory. Therefore, insertion of the ventilation tube at the early stage is important, and the tube should be placed beyond the 10 years of age.
