 A 67-year-old woman was admitted to our hospital with the complaint of recurrent massive epistaxis from the right side. She had sinus surgery in the right side 45 years ago, and the inferior and middle turbinates were completely removed and the sphenoidal sinus was kept open. With CAG, bleeding was proved to be from an aneurysm at the cavernous portion of the internal carotid artery. Surgery was successfully performed by the trapping and coating.

 Otolaryngologists should keep it in mind that when rapid and severe epistaxis is observed it is probably originated from the internal carotid artery, and if the normal anatomical structures in the nasal cavity are removed and sphenoidal sinus is open, the internal carotid artery will be most susceptible to the injury.
