外傷性仮性内頸動脈瘤の1例

Traumatic Pseudoaneurysm of Extracranial Internal Carotid Artery: Report of a Case

I．はじめに

　頭蓋外における内頸動脈瘤は，比較的まれな疾患である。1980年，永田ら^1）は本疾患に関する過去20年間の文献を渉猟した結果，自験例を含めて81例を算することを報告した。その後，耳鼻咽喉科領域では，高田ら^2）の1例，Laneら^3）の10例が報告され，その報告例は次第に散見されるようになった。今回われわれは，術前診断が困難を極め，しかも術後の嚥下障書に対して難渋した挙句，喉頭全摘除術を除儀なくされた外傷性仮性内頸動脈瘤の症例を経験したので，その臨床経過の概要を述べるとともに若干の考察を加えて報告する。

　A 51-year-old male complained of a painless swelling in the right parapharyngeal region and neck. There were no pulsation of the soft mass in the right tonsillar region, and no evidence suggesting of an aneurysm in cervical arteriography.

　Under general anesthesia, the tumor was extirpated en bloc following the ligation of the right internal carotid artery.

　The pathological findings of this tumor was shown as internal carotid pseudoaneurysm.

　Total laryngectomy was finally performed, because the marked disturbance of swallowing continued for 6 months after the surgery.
