上顎洞遊離骨片の2症例—遊離骨片発生病理に関する考察

Two Cases of Isolated Bone Fragments in the Maxillary Sinus

I．はじめに

　上顎洞内の遊離骨片は，副鼻腔根本手術の際に偶然に発見されることがあるが，手術件数に比して発見頻度は少ない。1893年にZuckerkandl^1）により本症が記載されてから，とくにその成因の面から注目されてきた。従来の報告では，粘膜由来説，骨由来説，含気化異常説など遊離骨片の成因に関しては，多数の仮説があり^2,3），炎症が根本原因とされている。しかしながら，これら従来の仮説は想像的なもので，遊離骨片の成因に関しては未だ不明の点が多い。

　最近筆者らは，孤立性および多発性と各々異なった形態の上顎洞遊離骨片の2症例を経験した。これらの2症例の遊離骨片および上顎洞粘膜を病理組織学的に検索したところ，遊離骨片の発生に関係すると考えられる従来の報告にはない知見が得られたので，遊離骨片の成因に関して若干の考察を加えて報告する次第である。

　A 29-year-old female was found to have an isolated bone fragment in each side of the maxillary sinuses. The other was a 34-year-old male with multiple isolated bone fragmants in the right maxillary sinus. These isolated bone fragments were found during radical operation for chronic sinusitis.

　The histopathological examination revealed that tiny bone fragments at several stages of bone development were embedded in the maxillary sinus mucosa of these patients. This finding suggested that the isolated bone fragments were originated inside the maxillary sinus mucosa.
