鞍鼻，埋没耳を合併した無汗性外胚葉形成不全症

A Case of Anhydrotic Ectodermal Dysplasia with Multiple Craniofacial Anomalies

I．はじめに

　無汗性外胚葉形成不全症は，先天的に汗腺の欠如あるいは低形成のためにおこる低耐熱性に加え，寡毛，乾燥といった皮膚症状のほか歯牙の形成不全，鞍鼻，厚い外翻した口唇など特微的な顔貌を呈し，萎縮性鼻炎や，稀に知能発育障害などを合併する疾患である。患者は小児科，皮膚科，歯科を受診することが多く，耳鼻咽喉科を受診することは極めて稀である。最近，鞍鼻，萎縮性鼻炎に加えて埋没耳を合併した無汗性外胚葉形成不全症に遭遇し，鼻および耳介の形成を行ったので，症例を呈示し，手術時の問題点を中心に若干の考察を加えて報告する。

　A 29-year-old female with anhydrotic ectodermal dysplasia accompanied with multiple congenital malformations such as saddle nose and bilateral pocket cars. The histological examination of theskin of the abdominal wall and the axillary region revealed aplasia of the sweat glands.

　Insertion of a silicone prosthesis for rhinoplasty, and reconstruction of the auricles using pedicle and free skin flap were done under general anesthesia. A special care was taken for control of body temperature during the operation and the postoperative course since hypothermophilia was confirmed by the surface thermometric investigation of the whole body.
