骨・軟骨形成を伴った鼻茸について

Nasal Polyp with Bone and Cartilage Formation

I．緒言

　鼻茸は日常の臨床においてしばしば遭遇する疾患であるが，鼻茸が大きくなり外鼻を変形させるに至る症例は少なく，ましてその大きな鼻茸組織中に骨・軟骨が形成されているような症例は極めて稀である。最近我々は鼻腔から上咽頭にまたがる巨大な鼻茸で，かつ骨・軟骨形成を伴った症例を経験した。

　Vogel^1）やEggstonとWolff^2）は慢性炎症のある場合，鼻・副鼻腔粘膜の結合織層の間葉系細胞が骨や軟骨に化生する事実を報告しているが，実際にはそのような症例の報告は非常に少ないようであり，また骨・軟骨化の機転について詳細な説明ばなされていない。一方，鼻茸の基質にみられる問葉系細胞は最近注目をあつめるようになり，この細胞が鼻茸の性状に大きな影響を与えるとされている^3,4）。

　A case of nasal polyp with bone and cartilage formation in its stroma was reported.

　A 21-year-old man complained of nasal obstruction in the left side of 6 years' duration. The physical examination revealed a large nasal polyp occupying the nasal cavity and epipharynx. X-ray examinations showed no bone destruction.

　Surgical treatment was done, and it was found that the mass originated from the lateral wall of nasal cavity and contained bone and cartilage. Pathological examination showed intramembranous ossification as well as endochondral ossification.

　These findings were briefly discussed.
