顔面に腫脹をきたした急性涙嚢炎の1例—乳幼児上顎骨骨髄炎の原因として

Acute Dacryocystitis with Swelling of the Face: Report of a Case

I．はじめに

　いわゆる乳幼児上顎骨骨髄炎は，乳幼児の鼻・副鼻腔，上顎骨の解剖学的特徴が大いに関与するために重篤な病像を生ずる疾患であり，その診断，治療には十分な配慮が必要である。また，本症の感染経路は，波及性感染と血行性感染に大別されるが，感染経路の推定は炎症の早期でないと困難であるといわれる。今回我々は，生後47日目の男児で，急性涙嚢炎から本症を生じた1例を経験したのでここに報告し，いわゆる乳幼児上顎骨骨髄炎の原因としての急性涙嚢炎の存在を強調した。また，新生児涙嚢炎（先天性鼻涙管閉鎖）に関して文献的に考察を加えた。

　A 47-day-old baby developed neonatal maxillary sinusitis due to acute dacryocystitis. A culture study from purulent nasal secretion showed multi-antibiotics-tolerant coagulase positive staphylococci. Use of appropriate antibiotics and incision of canine fossa led to the excellent relief of the symptoms. We emphasized that acute dacryocystitis could be the cause of neonatal maxillary sinusitis.
