声帯多形性腺腫の1症例

Pleomorphic Adenoma of the Vocal Cord: Report of a Case

I．緒言

　Pneolnorphic adenomaは上皮細胞の増生とともに間質成分の特徴的な存在様式により上皮細胞と非上皮細胞成分の双方の腫瘍性増殖と考えられたところからBillroth^1）が"Mischgeschwulste"と呼んで以来，永くmixed tumorとされてきたものである。この腫瘍は主として大唾液腺に発生し，次いで小唾液腺に多く見られる。また同様の組織所見を呈し，いわゆる唾液腺型混合腫瘍と呼ばれるものが気管支腺，乳腺，涙腺，および汗腺などにも発生することが知られている。しかし，この腫瘍が喉頭に発生した例は極めて少なく，文献周上Sabriらの集計によって19例が報告されているとされるが，中でも声帯部分は解剖学酌にもmucous glandの存在しない部位であるために例外的にしか報告されていない。我々は62歳男性の右声帯に発生した本腫瘍を経験したので，組織所見を中心に若干の文献的考察を加えて報告する。

　A pleomorphic adenoma was removed successfully from the vocal cord of a man suspected to have a mesenchymal tumor. Microscopically, the tumor was composed of pleomorphic or mixed appearance, epithelial tissue intermingled with areas of mucoid, myxoid and chondroid appearance. These histological findings were quite similar to the usual pleomorphic adenoma of salivary gland origin. This case was the third case of the pleomorphic adenoma of the vocal cord in the literature in Japan. Previous two cases were incomplete or simple type of "mixed tumor of the salivary gland" From this point of view, the present case was the first typical case of pleomorphic adenoma of the salivary gland type.
