　はじめに

　耳鼻咽喉科領域における多形性腺腫（以下，pleomorphic adenoma）はおもに大唾液腺，特に耳下腺に好発し1％〜6％の頻度で悪性像を呈することがあるといわれているが^1），ほとんどが良性である。この他にも口腔内や咽頭，口唇部の小唾液腺由来と思われるpleomorphic adenomaの報告例はあるが，喉頭，特に喉頭蓋における報告例は極めて稀である。今回，著者らは初診時呼吸苦を主訴とし，声門を確認できなかったので悪性腫瘍との鑑別をも要した喉頭蓋原発のpleomorphicadenomaを経験したので若干の文献的考察を加えて報告する。

A 73-year-old man developed a “lumpy sensa-tion” in the throat 6 months ago and respiration had gradually imparired duing lying down for a month. Laryngoscopy revealed a large mass on the laryngeal surface of the epiglottis. Pleomorphic adenoma was suspected after biopsy under general anesthesia and pharyngotomy was performed and the tumor was removed. After the further histo-logic examination, the tumor was diagnosed as a pleomorphic adenoma arising from the laryngeal aspect of the epiglottis. The patient had no evi-dence of recurrence more than two years.
