突発的な高度難聴を初発症状とした3疾患について

Three Cases of Sudden Hearing Loss

I．緒言

　いわゆる突発性難聴は耳鼻咽喉科医がしばしば遭遇し，実際に治療を担当する機会の多い疾患である。しかし，突発的な高度難聴をきたす疾患は数多くあり，現病歴と初診時の聴力像のみで安易に診断を下してはならない。今回われわれは，突発的な高度難聴を主訴として来院し，初診時には突発性難聴が疑われたが、諸検査によって原因と思われる疾患が判明した3症例について報告する。

　Three patients whose initial symptom was sudden hearing loss were diagnosed as having metastatic cerebellopontine angle tumor, infarctionof anterior inferior cerebellar artety and chronic myelogenic leukemia.

　Meticulous history taking of illness, neurotological examinations (gaze nystagmus test, eye tracking test and optokinetic pattern test) and hemo-analysis were helpful to differentiate these diseases from so-called sudden deafness.

　We emphasized that all patients who complain of sudden hearing loss have not so-called sudden deafness, and they must be examined thoroughly to rule out symptomatic sudden deafness.
