バセドウ病と甲状腺癌合併症例

A Case of Thyroid Cancer with Basedow's Disease

　I．はじめに

　バセドウ病は甲状腺機能亢進症の一つで，最近は自己免疫疾患であるとの考え方が強くなっている。その治療法も，抗甲状腺剤の内服療法やアイソトープ（¹³¹I）による治療の他に適応を選んで手術療法も行われる。パセドウ病は女性に多い疾患であるが，甲状腺癌もまた女性に多い疾患であり，進行が遅い分化型腺癌がほとんどで，他の癌とは少し違った考え方をしなければならない。分化型腺癌の治療法としては手術療法が一番安全である。放射線治療は分化型腺癌にはほとんど無効であるとの報告が多い。

　今回われわれは甲状腺機能異常を示すバセドウ病を元来有し，さらに臨床的に甲状腺癌の合併をみた稀な症例を経験し，治療したので報告し，両者の合併について文献的考察を行った。

　A 21-year-old female with thyroid carcinoma with Basedow's disease is reported. Hyperthyroidism was treated with antithyroid medication pre-operatively and subtotal thyroidectomy was performed under general anesthesia.

　Histologically papillary adenocarcinoma was diagnosed.

　After the operation thyroid function became within normal range with no recurrence of malignancy. In this report was reviewed coexistence of hyperthyroidism and thyroid cancer.
