鼓室成形術における保存鼻中隔軟骨の使用経験

Use of Septal Cartilage Homografts in Tympanoplasty

　I．はじめに

　鼓室成形術で，外耳道後壁の作製・補強や，鼓膜，コルメラの形成材料として，しばしば軟骨が利用されている。軟骨としては，耳介軟骨，耳珠軟骨，鼻中隔軟骨などが報告されている。筆者らは，細工がし易く，常備しておくことにより必要時に直ちに使用できる保存同種鼻中隔軟骨を，鼓室成形術に頻用し，好成績を得ている。以下，その使用方法を中心に報告する。

　Usefulness of septal cartilage homografts in tympanoplasty is reported. Cartilage was used as a block or disc which was micro-sliced (0.1-0.5mm thick) with Dermatome for posterior wall reconstruction of the external auditory canal, prevention of the retracted ear drum and as material of columella. No postoperative infection and extrusion of the cartilage were observed in our cases. Use of the preserved cartilage, expecially micro-sliced cartilage disc, could be utilized more extensively in middle ear surgery.
