咽頭側壁憩室・pouchについて

LATERAL PHARYNGEAL DIVERTICULUM AND POUCH

　I．はじめに

　咽頭後壁の下咽頭収縮筋と輪状咽頭筋との空隙（Lannier-Hackermanの3角）より発生する憩室は，Zenkerの憩室として知られ，詳細な観察がなされている。一方，咽頭側壁より発生する憩室の記載は乏しく，散発的に報告されているにすぎない。その名称についても報告者によりまちまちで，lateral pharyngeal pouch，lateral pharyngealdiverticulum，pharyngoceleなどと呼ばれている。

　筆者らは咽頭側壁憩室10例，咽頭側壁pouch 7例を経験したので症例を紹介し，その臨床像，診断，成因などについて検討を加えたので報告し，ご批判を仰ぎたい。

　Seventeen cases of lateral pharyngeal diverticulum and pouch are reported. Two of them arose from the tonsillar fossa and the remaining 15 arose from the pyriform fossa. The classifications, pathogenesis and radiographic characteritics of these lesions were discussed and the literature was reviewed. The authors emphasized that complete survey of the primary causes which might have increased the intrapharyngeal pressure while swallowing was attempted is important when lateral pharyngeal diverticulum or pouch was found.
