餅誤嚥窒息死の考察

CASES OF ASPHYXIAL DEATH BY RICE CAKE (MOCHI)

　I．緒言

　餅を誤嚥して窒息する例は決して稀なものではないが，実際にこれらの症例が医療機関で診療の対象となることは甚だしく少ないと考えられる。岩手県警の資料によると，昭和54年1年間に岩手県内で発生した餅誤嚥窒息死は16例であるが，24時間耳鼻科医がon dutyで勤務している県内唯一の病院である岩手医大付属病院に救急車で運ばれて来た例は1例のみである。このことは，餅の誤嚥が多くの場合直ちに窒息の原因となり，したがって，ほとんどが即死に近い状態で生命を失うために，一般には自宅などの事故発生現場で結着がついてしまい，病院に搬送されないことを意味していると思われ，止むを得ない実情といわなければならない。また後述する症例が示しているように，たとえ比較的短時間内にセンター病院などに搬送されたとしても，生命を救い得る状況にはないというのが現状であろう。

　Six cases of air way foreign body of rice-cake (mochi, a Japanese traditional cake made of pormded steamed rice) were reported and the results were summarized as follows.

　1) All patients were over 65 years old, and among them four had been suffered from various neurological abnormalities.

　2) Each of them was carried by an ambulance car to our emergency department within 15 to 45 minutes after accident but all of them could not be saved from death.

　3) It was concluded that only re-establishment of the respiratory tract at first-aid immediately after the aspiration of a foreign body might save their lives.
