舌咽神経痛の8症例—疼痛発作時に失神発作を併発した1例と口腔咽頭法による切除術の経験

EIGHT CASES OF GLOSSOPHARYNGEAL NEURALGIA: CLINICAL SYMPTOMS AND THE EFFECT OF TRANS-TONSILLAR SECTION OF THE NINTH CRANICAL NERVE

　I．はじめに

　舌咽神経痛は稀な疾患といわれている。これには特に原因となるものがみあたらず，舌咽神経支配領域（舌根部，口蓋扁桃，咽頭，中耳）に疼痛の起こる特発性舌咽神経痛と，脳腫瘍の他，咽頭腫瘍^1），動脈瘤^2），茎突舌骨靱帯の石灰化^3），茎状突起過長症^4）など原因があって，この領域に疼痛の起こる症候性舌咽神経痛とがある。

　筆者らは前者の特発性舌咽神経痛と診断された症例を，昭和51年1月から昭和54年10月までに8例経験したが，そのうち1例は疼痛発作時に失神発作を反復する稀な症例であった。これらに口腔咽頭法^5,6）による舌咽神経切除術を試み，良好な成績を得たので報告する。

　Eight cases of glossopharyngeal neuralgia which treated from January 1976 to October 1979 were reviewed. Interesting symptoms were observed intwo patients : a 67-year-old woman had syncopal attacks accompanied with glossopharyngeal neuralgia and a 52-year-old man had trigger zones in the external auditory canal, the auricle and the base of tongue.

　Trans-tonsillar section of the glossopharyngeal nerve was performed on all eight cases and resulted in permanent relief of pain in all cases until the time of this report. This simple and less invasive surgery should be the first choice of surgical treatment when the trigger zone is defined.
