インピーダンス法で耳管機能はどこまでわかるか

RELIABILITY OF IMPEDANCE METHODS IN DIAGNOSIS OF EUSTACHIAN TUBE DYSFUNCTION

　Ⅰ．はじめに

　中耳疾患の予後を大きく左右する耳管の機能を，客観的に測定しようとする試みは，従来から多くなされてきた。なかでも近年普及してきたインピーダンスメーターは取り扱いが簡便なことから，この器機を用いた耳管機能検査法が種々考案報告されている。しかし，これらの検査法から得られる情報には一定の限界があり，その信頼性に問題のある検査法もある。

　そこで本稿では，従来の代表的インピーダンス法の信頼性を再検討し，各検査法の問題点を明確にしようと試みた。

　Evaluation of 3 different methods of impedance tests for tubal function was carried out.

　1) Diagnosis by single tympanometry was found to be unreliable, because the obstructive tube produced any type of tympanogram.

　2) Tympanometry before and after tubal inflation by Valsalva's maneuver indicated the dynamic process of tubal opening. However, the disadvantage is that the measurement of tubal obstruction in a patient showing type B tympanogram was difficult to perform.

　3) By impedance measurement during tubal inflation, the diagnosis of tubal dysfunction was made by a rather simple pattern of compliance. Its disadvantage would be difficulty of quantitative judgement of tubal function.
