鼓膜弛緩症

FLOPPY TYMPANIC MEMBRANE

　I．緒言

　最近滲出性中耳炎を対象とした当科中耳外来で鼓膜が極度に内陥し，しかもゴムひもが伸び切ったような状態に弛緩し，ツチ骨やキヌタ・アブミ関節が浮き彫りになり，一見癒着性中耳炎を思わせる症例がみられる。ところが，これらの症例に耳管通気を行うと逆に鼓膜は，非薄な感じで紙風船のごとく外耳道側に膨隆してくる。第1図は，通気前の右鼓膜である。第2図は，通気後の同症例右鼓膜で，特に後上象限部に膨隆が著明である。どうしてこのような病態を呈するのであろうか，筆者らはこれらの症例を，一応鼓膜弛緩症（仮称）と命名し，この疑問を解決するため臨床的観察を行った。

The term of "floppy tympanic membrane" is given by the authors to the state of the tympanic membrane which shows bulging such as a balloon after inflation of the auditory tube was done. There was no fluid in the middle ear. Abnormal occlusion of the tube was not recognized. Most bulging part of the drum was posterosuperior quadrant. The floppy ear drum possibly develops into adhesive otitis and cholesteatoma.
