頭頸部悪性腫瘍の宿主反応性に関する研究—OK-32投与症例を中心に

IMMUNO-CHEMOTHERAPY FOR MALIGNANT TUMORS IN THE HEAD AND NECK: HOST IMMUNE RESPONSES AFTER OK-432 APPLICATION

　I．緒言

　頭頸部領域の悪性腫瘍に対する治療は，外科療法，放射線療法そして化学療法が中心になつている。しかし制癌効果は期待できるが宿主の免疫能とりわけ細胞性免疫能が低下し，total cell killがなされない以上再発は必至であるとされている。このようなことから癌化学療法において，宿主免疫能の状態がその制癌効果と密接な関係があることが注目されている。すなわち，悪性腫瘍の治療に際し制癌剤の殺細胞作用および放射線療法による制癌効果は期待できるが，一方では宿主の免疫能とりわけ細胞性免疫能の低下が避けられない。そこで宿主免疫能を賦活することにより化学療法などによる免疫抑制作用をできるだけ防止し，さらにその制癌効果を高める方法が求められ試み始められたのが免疫療法，免疫化学療法である。非特異的免疫能を賦活する薬剤として，欧米ではBCG，levamisoleなどが中心となつているが本邦では溶連菌制剤OK-432が開発されている^1）。OK-432単独投与による免疫能の変動を観察するには同一癌腫，同一病期で検討するのが理想であるが，現在までの報告から癌患者に単独投与法を選ぶことはできない。われわれは症例数も少ないので，各種治療法とOK-432併用投与のおよぼした宿主への生体反応として検討したので報告する。

　After administration of streptococcal preparation (OK-432) to patient with malignant tumors in the head and neck, the cell-mediated immune responses and effects of the immuno-chemotherapy were investigated. The results obtained were as follows.

　1) Nine out of 17 patients were treated successfully by the administration of OK-432 and cobalt 60 irradiation.

　2) The increase of the white blood cell count, the elevation of r-globulin and IgG rates were noted in blood in most instances.

　3) There was a marked difference of the erythrocyte sedimentation rate (ESR) between the patients with good and bad prognosis. Low ESR level was obtained in the patients with good prognosis.

　4) There were no serious side effects throughout of the use of the streptococcal preparation (0K-432). The only noticeable symptom was fever observed in 40% of all patients.
