青色鼓膜に対するリニアック電子線照射経験

ELECTRON-BEAM OF LINIAC THERAPY FOR GLUE EAR

いわゆる青色鼓膜は古くより知られているが種種の治療に抗して治癒しにくい疾患である。本症の病理組織学的所見は中耳鼓室より乳突蜂巣にかけて広範囲に及ぶコレステリン肉芽腫を主とした粘膜病変とされている。本症の成因については中耳粘膜の分泌機能亢進あるいはこれに伴う排泄障害，耳管機能障害その背景にはアレルギーが関与しているとする説があげられている。

　治療に関しては十数パーセントの症例は種々の治療に抵抗して年余にわたる難治例があるといわれ，1）肥大したアデノイド切除，口蓋扁桃摘出あるいは耳管咽頭開口部および周囲のリンパ組織の除去，放射線の利用。2）鼻副鼻腔疾患，上気道，上咽頭疾患に対する処置。3）耳管通気，耳管内薬剤注入など。4）鼓膜切開，穿刺，チューブ挿入，乳突洞削開術などが行なわれ単一の治療法では治癒を期待することは困難であるといわれている^1）。

　Two cases of blue ear, in which each was treated by conservative method in one, and by mastoidectomy in the other, were both given irradiation treatment by use of electron-beam of 10 MeV of liniac. The promontory area was irradiated through the external auditory canal by a one 2 cm in diameter. Case 1 and case 2 were given 600 and 1, 500 rads respectively. After irradiation the blue ear drum made complete recovery.

　The hearing acuity of case 1 is found to be in normal limits at present four years after the treatment, in case 2 the recovery is slight because the ossicular chain had been damaged by the long lasting glue ear. There were no side effects noted.

　The authors recommand that electron-beam therapy may be worth trying for curing in cases hard curing glue ears.
